Atrial Fibrillation: Achieving Control

The AFFIRM trial revealed that a strategy of rhythm control with conventional antiarrhythmic drugs (AADs) conferred no outcomes advantage over rate control in atrial fibrillation (AF). Despite these results, the decision to select either or both strategies remains difficult. Recent data from clinical trials show that agents with both rhythm- and rate-control properties are effective at maintaining sinus rhythm, avoiding cardiac and extracardiac side effects often linked with traditional AADs, and improving clinical outcomes, cardiovascular events, and hospitalizations.

In an effort to help physicians apply the latest strategies in AF to patient care, Dr. Camm discusses the pros and cons of selecting rate- and/or rhythm-control strategies for patients with AF; Dr. Naccarelli discusses the rationale for aggressive or lenient rhythm control in AF and appropriate candidate selection; Dr. Hohnloser discusses the prognostic significance of hospitalization in AF and the need to evaluate rate- and rhythm-control strategies to reduce their occurrence; and Dr. Waldo reviews the latest clinical trial results in the management of AF and discusses how these data may impact future treatment guidelines.
